What does this information mean?

It's only accessible at the last stage of The Screaming Mace . Only one in the group need to be at that stage to zone the whole group in.

There is only one small chamber with 4 totems. Clicking the correct one will spawn Gorfuzod the Keeper , clicking any other one will spawn 4 harbinger of Fear . Examining Congealed Blood Sphere will highlight the proper idol.

Once the Keeper is dead, you can pick up the Screaming Mace to finish the quest.
